How can we add an Image or Logo to the Oracle Evaluations?

We have a requirement to add an image or logo to the Evaluation window. I need to include a logo in the areas mentioned below. Is there a possibility to add images in these sections?

Navigation to the Evaluation window: Me > Learning > Course (name) > Evaluation

I have created the questions by using the navigation below

Setup and Maintenance > Task list > Search > Questionnaire Library > Subscriber (Learning Evaluations) > Create

I have created the questionnaire template by using the navigation below

Setup and Maintenance > Task list > Search > Questionnaire Template > Subscriber (Learning Evaluations) > Create
