Document of Record +add button unresponsive

Summary:

We are running into an issue where our +add button in document of record is unresponsive that start about a week and half to two weeks ago. We have tested with the seeded HR specialist Role, Cleared Cache, and tried in Incognito Mode and the issue still persists. We just received December CWB patch on Friday (12/19/2025) and this didn't resolve the issue. Is anyone else running into this issue or what can we do to try and resolve this? Image has been attached to show which +add button I am referring too.
