Email Based approval in OIC Process Automation Failing

We have configured email-based approvals in Oracle Process Automation (OPA). We are facing an issue when the same task is acted upon first from the application UI and later from the email notification.

Scenario and Steps to Reproduce:

  1. A user receives an FYI email notification for a task that includes an “OK” action.
  2. The user logs into the application and approves the task from the application UI.
  3. The PCS process is completed successfully.
  4. The user later opens the original FYI email and clicks the “OK” action.
  5. The system sends another email.
  6. After a few minutes, the user receives a TASK APPROVAL FAILURE email.
