Intermittent Failure When Running Requisition Status Inquiry Agent

We have noticed that when we test our agent by clicking the Run button, it behaves inconsistently—sometimes it works correctly, and other times it shows the following error:
"Sorry, the assistant is unavailable right now. If the issue persists, contact your help desk."
For example, our Requisition Status Inquiry agent retrieves requisition details by passing the Preparer name (obtained through the getUserSession tool) as the q parameter. Whenever we run the agent, getUserSession consistently returns the correct username, which is then passed to the Requisition tool. However, despite using the same Preparer name, the agent sometimes returns results and sometimes fails.
