Oracle Learning Course Assessment Report

Does anyone have a BIP Report from the Learning Modules containing data similar to the following? So far, we haven't been able to find all the needed data elements. A sample report from our previous LMS is attached. Thanks for any assistance.

Our Learning Department Administrator has requested an aggregated report showing the following information for SCORM Quiz Types. The idea is for the report to show potential weaknesses in our team members and/or educational content.

For a Given Course:
