Terminated Employees with Active User Account

Employees who have been terminated still have an active user account.

We recently noticed that some user accounts for terminated employees are still showing as active and the issue appears to be random. We haven't been able to identify any pattern or common factor amount these accounts.

I read a few similar posts and many mention that this can happen when the auto-provisioning role is not tied to an active HR Assignment Status. However, if that were the cause, wouldn't ALL terminated user accounts stay active instead of only some of them?
