When and Why train stop is showing blank page during pending worker approval

Summary:

Once the manager submit the Pending Worker Record for approval, the HR click on the 'Edit' option in approval workflow notification.

This open the Add Pending worker page. In the When and Why page no information / fields are shown and is blank. Other pages in the Pending worker train stop shows all the info.

This was working correctly and only after 26A upgrade this issue is happening. Hence wanted to check if anyone else is facing a similar or same issue.
