BI Publisher report based on RTF template not displaying from all joined datasets

I have a data model with a "primary" dataset that joins to 3 other datasets as "lookups" for additional data.

I have created an eText RTF template and am getting the data displayed for the primary record details, and the first lookup dataset. However, after the data from the first lookup is displayed, it starts the next row/record and doesn't output the data from the second and third dataset. I have tried various iterations of no nesting and multiple combinations of nesting the lookup <LEVEL> <END LEVEL> elements and have not been able to get the second and third dataset data to display.
