Supervisor Report

Does anyone know how to run a report that will include supervisors first and last name and users that are assigned to that supervisor? I also need to have users first and last name, department & divison (our two custome fields). Regular Supervisor Report runs the report  based on username, but it does not give you the ability to add suerpvisors and users first and last name.
