Oracle password reset issue

Hi,
I am currently facing this issue with one of my clients. Users are being prompted to change their passwords, but when they attempt to do so, they receive the error shown below and are unable to complete the password reset process. So far, we have observed this with two users over the past two days.
As a workaround, the client admin was able to reset the passwords through OCI.
Has anyone else encountered this issue recently? Is this a known Oracle issue, and are there any recommended fixes or updates from Oracle? The error message is attached below.
