Agent not working once attached to Smart Action

I have built one workflow agent , it is working perfectly fine on debug mode on ai agent portal but I attached to Smart Action on Cx object it is not returning any output and no response after that.

Not sure what will be the reason, put a evaluation also bit no error generated.

Anyone faced similar or any leads to fix this
