Agent Failure in Agentic App Builder — affects all agents including Oracle demo apps

We are getting Agent Failure — This advisor failed to return results in the Agentic App Builder. We have implemented the OraMessageHint routing pattern as per the Configure Agents for Agentic Apps blog post. The Switch node is correctly routing on $context.$app.$OraMessageHint.

We noticed the same Agent Failure appears in Oracle's own demo apps in the same instance. Is this a known issue in Vision demo environments?
Is there an instance-level configuration or setup step required to enable Agentic Apps to work correctly?

Environment: Oracle Fusion Cloud Vision demo instance, AI Agent Studio, Agentic App Builder.
