Users unable to download attachments on approvals

Our 'field HR' people initiate a transaction, such as "Hire", and attach documents to the Documents of Record region on the transaction.
We have two teams that receive the approval request in sequence: 1)Workforce Planning and 2)Data Entry. Workforce Planning users Claim and then Edit the approval request in order to audit the entire transaction. On the Document of Records region, they download the documents that were attached by the 'field HR' representative. They compare the attachments with the transaction and then approve it, which forwards the approval to the Data Entry team.
Data Entry performs the same steps, but consistently can't download the documents that were attached. The user can see the document is attached, but they click the download icon and nothing happens.
