Role Mapping SQL Report

I need assistance in developing a SQL query that will generate a Role Mapping report, equivalent to what is configured in the “Create Role Mapping” UI page.
Requirement
The report should extract the following details:

• Role Mapping Details
  • Mapping Name
  • Description
  • Start Date / End Date
  • Status (Active only)
• Conditions (Mapping Criteria)
  I need to retrieve all condition fields shown in the UI, including but not limited to:
  • Legal Employer
  • Business Unit
  • Department
  • Job
  • Position
  • Grade
  • Location
  • Legislative Data Group
  • Assignment Type
  • System Person Type
  • User Person Type
  • HR Assignment Status
  • Assignment Status
  • Manager with Reports
  • Manager Type
