I am not able to add the product or ingredients to the the item formula.

I am not able to add the product or ingredients to the the item formula.
The Primary formula header has been created. But we are not able to add the product or ingredients to the the item formula using select and add items.
Could you please help on understanding why this is happening. Are we missing any setups?
