APEX authentication ORA-01403

Locked out of APEX App Builder with APEX_AUTHENTICATION.NOT_CONFIGURED / ORA-01403 no data found on WWV_FLOW_AUTHENTICATION, following a series of session expiration errors.


Content (please ensure you mask any confidential information):

Beginning approximately 1:00 PM CDT on August 8, 2026, sessions began failing intermittently and then an internal authentication configuration error appeared that has not self-resolved.

Timeline of errors observed, in order:

  1. APEX.SESSION.EXPIRED - "Your session has ended" immediately after successful login, reproduced across Chrome (normal + Incognito) and Microsoft Edge, after clearing all browser history/cookies and after a full system restart.
  2. apex_authentication.callback returning a completely blank HTML document instead of completing the login redirect.
