Error when running Redwood Perform Mass Change Item Class

Summary:

We have configured the Redwood: Mass Change Item Class however, after we have built the guided journey as instructed in the release notes we receive an error. The error comes into play when trying to complete the 3rd task Review and submit. I am getting this error:

What I did notice is that in the release notes it states a spreadsheet will be created to review and submit and I am not getting that spreadsheet.

I pulled the developer tools and noticed the code mentioned below. It appears that it is trying to add it to an existing change order. I never set that up?
