Purpose of Questionnaire in Intake Request Template (Fusion Procurement)

The documentation describes the questionnaire as optional, but we could not find a clear description of what it is used for and how the answers are processed. We would appreciate your experience on the following points:

1. What is the intended purpose of the questionnaire in an intake request template?
2. Where does the requester see and answer the questions, and where can reviewers or the intake manager see the answers?
3. Are the answers transferred to the outcome document (for example into fields or notes of the requisition), or are they only available on the intake request?
