Best Practice of opatch

I'm new to opatch. As I understand it will patch both binaries and database. Does that mean it is basically meaningless to do rman backup? Also does that mean will the rman backup prior to opatch will be useless?

What are the best practise of patching oracle database? Do you guys do a full server backup before you perform a patch or you just patch it? I find that performing a full server backup takes too long but patching directly seem so dangerious.
