Record History is showing updates by users without access to purchase orders

 We have a purchase order in our Colombia Organization and the record history shows that a user from our United States Organization updated the purchase order.  The US user does not have access to any Colombian Responsbilities and the user stated that they didn't do anything with the Colombian Purchase Order.  We ran a script and found that this seems to happen quite a bit. 

Has anyone else run into this issue?
