Cannot Apply Monitoring Template - Internal error occurred

I am trying to create a Monitoring Template and apply it to a database instance.  I've tried creating a template with just 1 policy, but every time I apply it to an instance, I get "Internal error occured" on my Apply Operations page.  Any ideas?
