User Defined Policy - Doesn't show violation

 I have created a User Defined Policy and successfully tested (showed violation during creation) on an instance.  However, when I add this policy to the same instance, I cannot get a violation.  If I add the Policy to a Policy Group and run a scan, I see the violation on the instance.  How do you get User Defined Policies to appear in alerts or Policy Violations?
