Requistion Approval Process Fails With "No Approver Found" When Approver Edits Requistion

We came across this issue recently where Requistion Approval Process Fails With "No Approver Found" When Approver Edits Requistion. This did not occur untill last month and started happening suddenly so applying a patch is not solution. Only change we did was made some changes to approval hierarchy. Please let me know if some setup change is causing this.
