Can we see the IP address of all users online for an instance??????

    How to see user IP address for all connected users???I can only see the HOST NAMES using " select TERMINAL from v$session; "....but some of the terminal names read as " unknown " and some show just a blank ...why ???
Will using  " sys_context" help solving problem.....how???
