11g After Expiring Password, User Not Prompted for New Password

 I recently installed 11g Enterprise, and created several users successfully. However, when I attempt to create a user and expire the password (so that the user will have to create his own when he first logs in) the user is not prompted. For example, if I try to log into the EM console with the username and original expired password, the message that displays is "Error: Login Operation Failed." If I reset the password, the user can log in. If I expire it again, the same error message is displayed. The account remains open, and not locked, following the expiration,
