PRs Stuck in Workflow

 We are using Oracle Version 11.5.8.  We use Position Approval Hierarchy to approve our created PRs. We had some issues on prs being stuck in WF but not as often as today.  Users has been receiving an error after a pr is forwarded for approval or attemp to approve.  The error message is 'Web page can not be found' and if this happens, the prs being forwarded for approval or approved get stuck in WF (not a 100%).  Does anyone experience this issue?  If you have, how it was resolved?  Thanks for your help in advance.
