Referencing published HR questionnaire fields for use in alerts

  There is a requirement to configure several alerts based on probation reviews:
- 16 days after hire: send alert to employee's manager to process review - 3 months: send alert to employee's, manager to process review - 6 months: send alert to employee's manager to process review - 9 months (for extended probations only): send alert to employee's manager to process review The only place where extended probation is recorded is on the 6 months review form (designed using HR questionnaire) where there is a field to indicate this. Question is how do we reference fields from published HR questionnaires in order to use in the alert SQL statement?
