Issues in Service module after upgrading to 12.1.1

We have recently upgraded our Oracle E-Business from R12.0.4 to R12.1.1.  After the upgrade, we are no longer able to create Service Requests using the OAF version.  Using the Forms version works fine.  Has anyone experienced this issue, and if so did you have a solution?  I have an Oracle TAR opened for this issue, but wanted to post this to this forum just in case some has a solution.  TIA.
