Not able to create County or City Based Taxes

I am not unable to create county or city based Tax Jurisdiction. The system throws an error saying the jurisdiction already exists for this combination. The Jurisdiction for the sate was just created by me and no other jurisdiction exists for the Regime. This is a newly installed R12.1.1 instance.
I created a TCA geography with the standard structure provided in fresh instance. I have not created any zones.

Can someone please explain how I cam resolve this issue. Any method to debug this would be of great help!!!
