ASDLs executed twice but listed once in OCA

I have seen some unacceptable behavior from ASAP. ASDL got executed twice in the JNEP logs but in OCA client it shows once. 1st time ASDL executed successfully and performed some transactions on Network and when it executed second time it failed as 1 occurrance was successful.

If someone has seen this behavior and knows the solution, please let me know. This is urgent.
