Fuel Surcharge CSV Files

We are currently updating our Fuel Surcharge rates manually every month for both our TL and LTL rates. This process can take the users a few hours when done manually.

Can someone supply me with a sample CSV file that we can populate with our fuel surcharge data and upload in one file to OTM? Or if there is documentation on how to do this, can someone point us in the right direction?
