Unicode problems

We want to upgrade our database to 10.2.0.4 but postponed this several times because some of our databases are configured with Unicode and I've read this might cause problems . I have the following questions:
- has this already been resolved? I ask this because I cannot find any document on Metalink about this article.
- is an upgrade to 11.1.0.7 much more effort despite of the fact this is a major release?
