Importing PSR Data to the Affected Items Tab

I am trying to import data to the Affected Items tab or 'codes' as it appears on our screen, of a PSR.  I already have the PSR's in the system and now I'm trying to add the correct code to each one.  The three fields I have in my spreadsheet are the Item Number, the Code number and the Rev Code which I understand is required.

Can anyone help me with this?  It keeps giving me a Fatal Error with no detail on the error message.
