What is the relationship between /usr/tmp on the DB tier and $APPLPTMP on the App tier?

I'm an Apps DBA of a few years now, but I haven't found any clear documentation explaining the relationship between /usr/tmp on the DB tier and $APPLPTMP (set to "/usr/tmp") on the App tier.  My Concurrent Managers reside on the Application tier.  I see that some concurrent requests write temporary files into /usr/tmp on the DB tier -- how do these files get transferred back to the App tier?  What's the best way to purge /usr/tmp on either tier?
