Tax Exemptions for State, County or city

 We have a customer that needs to be exempted at the state level but taxed at the county and city level.  We have tried putting 100% at the State and 0% at the main exemption level and no tax is being added to orders.  We have also put 100% at the main exemption level and 100% at state but they are not getting chared for local taxes.  Any help as to what I am doing wrong would be appreciated.
