Workflow Issue - User Approved and it didn't take in AGILE

We are working within a certain workflow in Agile that was created by us.  We pushed the issue or change order into the next phase of the Workflow.  One Director went to sign off.  The 'History' tab showed she signed off but the workflow tab shows she didn't.  Any thoughts on this one.
