PO Workflow Approval Error Number 3

We periodically and seemingly randomly get the below POAPPRV workflow error:

The Document Manager failed with error number 3.

Error Number 3 = Unknown Error.
Contact your system administrator.

Error Messages:

There are no Oracle error messages given in the notification and it does not occur on every PO. We also cannot find a common thread in the PO's that do fail.

Has anyone seen this before and resolved the issue?
