requisition line used in two different PO lines

HI,

I see that all 3 lines on PO C are duplicate orders.

Requisition A Line 7 is on orders Y line  and also order C line 1 .
Requisition A Line 8 is on release B-15 line and also order C line 2.
Requisition A Line 3 is on release B-15 line and also order C line 3.

What may be the reason? I am looking for a root cause.
