Cannot enter Notes in iSupport while creating a Service Request

 Hello,

Iam trying to create a Service Request from iSupport but I don't have the option to enter the notes in Problem Detail Sections ..I did all the setups related to Notes in CRM Administrator (Source and Note Type Mapping), Bounced the Apache Servicer but still I cannot view the fields related to Notes under the Problem Details Section of ibuSRIdentifyProblem.jsp

Please suggest if Iam missing anything
