PO approval workflow faile for one PO

 Hi , We have an issue where our PO approval workflow has failed with following error

Retry Activity : POAPPRV

seleced Activity= PO Approved
Activity status =  Error
Activity Type =Notice
Notification subject = Error ORA-01403  no data found error Name : 100 Error stack.


I looked at PO and found the status for this PO is approved.

Can any one suggest what could be the reason of failure.
If it is failed then why the status has changed to "Approved".
