Logical standby fails with ora-01422 exact fetch returns more than 1 row (10.2.0.4)

 We have an Oracle 10.2.0.4 logical standby that is fed from an Oracle 10.2.0.4 RAC cluster.

The logical standyby failed on Friday with an ora-01422 exact fetch returns ...., the primary database continued perfectly.

The result was that the logical standby had to be rebuilt.

Anybody have any idea why this may occur, we are suspicious that it may have been a contention issue on live but have no firm evidence.

Searches on metalink etc. have failed to give us any help !
