Cannot view the template questions on ibuSRIdentifyProblem.jsp

 Hello,

I have a created a template with few questions but when I try creating a service request with the same template I cannot view the questions

which I have set for this template on ibuSRIdentifyProblem.jsp....I have also looked at the metalink note below but still cannot find the questions associated to the template while creating a servicve request..Can you please let me know if Iam missing anything
