What is the cause of the memory based planner failing with:

Could not find dest operation for OPM route transfer transaction id: 49686 , op seq num: 10 , to op seq num: 20 , supply id: 49686 , item id: 8002 , org id: 454 , instance id: 3011 , routing sequence id: -23453 , routing type: -23453 , supply type: 3. Please have MBP look into this data condition, as we could not find this operation sequence in Res Reqs. /u01/dev/apps/apps_st/appl/mso/12.0.0/bin/MSONEW
