Duplicate tasks in the time sheet tab

Hi, Did anyone experience duplicate tasks in the Timesheet tab? i've seen it on some users timesheet that almost all tasks have been duplicated. how can I remove the duplicate tasks? What caused it to duplicate? Any thoughts are appreciated. it happend to just one group of users in the organization and all of them have duplicate tasks in the timesheet.
