Agile Data Dictionary

  I am being asked to create a Data Dictionary or ERD (Entity-Relationship Diagram, which shows how the tables are related and what columns are used to join tables) from Agile.  Instead of starting from scratch, I was wondering if anyone has something like this already created and if you would be willing to share it.
