Unable to cancel a query in EBS 12.1.2 (ora-01001 whcn clicking cancel-query button)

Hi,

I found note 436537.1.  But i'm not able to apply patch  5754150 because of a conflict with patch 7379122. My forms installation is coming from EBS 12.1.1 DVD and the version is 10.1.0.5.0 (with some patches includes like 7379122)

Any idea ?
