Last Update date not in service request history

Recently I started noticing more often than not that the Last Update date on my service requests is more recent than the most recent history entry.
This is quite confusing because the Dashboard "Service Requests" regions show in a "Last Updated" column values like "Today" and if I try to review what changed, I am unable to determine what the update was, as such a waste of time.
The few times I tried to find out form the support engineer what was updated, they also couldn't say what changed.
