Upgradede Release 12.1 Instance is failing for logon - RW-50016: Error: - {0}

Hi,

I am upgrading our EBS R11i to EBS R12 (12.1.1). Just completed applying American English Upgrade Driver - u6678700.drv.

I am currently doing the Configure and Start Server Process in Finishing the Upgrade Section (page 3-8 in the Upgrade Guide Part #E14010-01). During Post-Install Check, Validate System Configuration is showing Failed for Login Page with the following error:
