Multiple Contacts and Tasks using PIM Sync

Has anyone encountered PIM Sync creating multiple records in Outlook for the same contact or task?  I have an instance where Outlook is showing three or four copies of the same contacts and tasks after a sync.  Any suggestions would be greatly appreciated.
