Custom Form Item Prompts Displaying, but Items Do Not Display (Screenshot Included)

I made a custom form from TEMPLATE.fmb, and it displays fine in forms builder, but when I run it, none of the items display. Only the item prompts.

Any ideas on this? What could I have goofed up that would cause that?
